Anybody got a template consent form (foreign trips)?

I haven't done this sort of thing for a while...

Does anyone have a specimen consent form for a foreign residential trip??

I'll e-mail our trips and visits co-ordinator at school and ask her if she has anything on file she can send to me that I'll then pass on to you.

Asked our trips and visits co-ordinator and she said we just use Local Authority ones along with the ones the travel company provide for us.  She hasn't got anything electronically but she's going to see if she can dig something out that I will try and post.
